Please have all paperwork signed and completed prior to my arrival in order to ensure optimal amount of time allotted to your wellness goals.

Please have all insurance information, medical records, photo Identification as well as any accident information with you (if applicable)

Initial consultations may take from 15-30 minutes or even longer due to case-to-case basis. Sessions can be scheduled after initial consultation if needed.
